    Ovarian Cystectomy Overview

An ovarian cystectomy is a surgical procedure to remove ovarian cysts, which are sacs on the ovary that often resolve without treatment. This procedure can be done laparoscopically (using a laparoscope for minimal invasiveness and easier recovery) or via open surgery (with a large incision), which is necessary if the cyst is large or cancer is suspected. Surgery is recommended if the cyst persists, grows, or causes severe pain. Ovarian cysts are commonly caused by hormonal disorders, ovulation, endometriosis, pelvic inflammatory disease, and cancer.

    Types Of Ovarian Cysts

Ovarian cyst types are as follows:
    • Functional cysts (harmless): These include follicular and corpus luteum cysts.
    • Non-functional cysts (benign or malignant): These include dermoid, cystadenomas, endometriomas, and Brenner tumors.

Preparation: The surgeon will discuss the surgery details, perform a medical history review and physical examination, and also suggest imaging and blood tests for a cyst. They may also discuss the preparation for the surgery, including diet, drinking, and stopping medications prior to the surgery.

During the surgery: An ovarian cystectomy involves general anesthesia and an intravenous catheter. The surgeon inserts a laparoscope through a small incision near the belly button and inflates the abdomen with carbon dioxide. Two to three small incisions are made in the lower abdomen to insert instruments and remove the cyst. The incisions are closed with dissolvable stitches. If laparoscopic surgery is not feasible, open surgery is performed through a larger abdominal incision to remove the cyst.

After the procedure: After surgery, the patient will be shifted to the recovery room, where the surgeon will monitor their vital signs. The patient may experience tummy pain, which should improve within days. It may take 1 to 2 weeks to resume normal activities after a laparoscopy or laparotomy. Testing results will be available in a few weeks.

Ovarian cyst removal recovery time: Recovery time for laparoscopic cystectomy is 1–3 weeks, with resumption of usual activities after 1-2 weeks. Open cystectomy recovery can last up to eight weeks. After the laparoscopic cystectomy, patients can return home within two to three hours, while open surgery may require an overnight stay.

Post-procedure care: Post-surgery care guidelines include:
    • Prioritize rest.
    • Take the prescribed pain medication.
    • Follow the doctor’s instructions for incision hygiene.
    • Avoid strenuous activity or heavy lifting.
    • Transition to a regular diet gradually.
    • Consult a doctor for excessive bleeding or foul-smelling discharge.
    • Schedule follow-up appointments.
    • Resume normal activities upon the surgeon’s consultation.

    FAQ's

Laparoscopic ovarian cystectomy is a safe and effective procedure with minimal invasiveness, smaller incisions, less tissue trauma, reduced pain, and shorter hospital stays compared to traditional open surgery, allowing patients to return home the same day.

In general, the type of surgery depends on the surgeon’s technique. Laparoscopic surgery is minimally invasive and typically returns to normal activities within two to three weeks. Open surgery is more invasive, requiring six to eight weeks of activity modifications and up to 12 weeks for full recovery.

The primary goal of surgery is to minimize ovarian damage through a minimally invasive technique, such as laparoscopy, which allows for more precise dissection and minimizes damage to healthy ovarian tissue. However, the surgeon’s experience and skill also significantly impact the removal of the cyst while preserving healthy ovarian tissue.

Following surgery, expect soreness around the surgical site for 24 to 48 hours, with walking encouraged based on energy level. Recovery is quick, with most patients feeling better within the first few days. Walking after surgery improves circulation, reduces pain, aids recovery, and boosts mood, preventing blood clots, promoting faster recovery, and promoting well-being.

Ovarian dermoid cysts may not require removal unless they become large, cause symptoms, or may become malignant, as recommended by the provider. If they are serious, then the surgeon often recommends various ovarian dermoid cyst treatment options.

Hemorrhagic ovarian cysts are usually harmless and resolve without treatment. However, ruptured or twisted cysts or internal bleeding may require emergency medical attention.
