Holmium laser prostate surgery (HoLEP) overview:

Holmium laser enucleation of the prostate, commonly called HoLEP, is a prostate surgery that is similar to an open procedure and does not require any incisions. This is an effective treatment option for the enlarged prostate, known as benign prostatic hyperplasia (BPH). It is a minimally invasive procedure to treat the enlarged prostate. HoLEP preserves tissue removed for laboratory examination for other conditions, including prostate cancer.

The procedure involves using a laser to remove prostate tissue that blocks urine flow, followed by cutting the tissue into easily removable fragments. HoLEP, like other prostate laser surgeries, offers faster recovery and symptom relief compared to traditional prostate surgery methods

    Procedure Name HoLEP
    Type of Surgery Minimally invasive
    Type of Anesthesia General Anesthesia
    Procedure Duration 1-3 hours
Recovery Duration Few days to few weeks
    HoLEP: Pre-Op & Post-Op Care

Preparation: Before HoLEP surgery, essential tests, including blood, urine, and possibly X-rays, ensure safety and effectiveness. Following this, the procedure begins under general anesthesia.

During the procedure: During the procedure, a resectoscope is used to separate the core of the prostate from its outer border using a laser-fiber instrument. The tissue is then removed from the body using suction, and the inner prostate tissue is cut into small pieces.

After the procedure: After tissue removal, the surgeon removes the resectoscope and inserts a urinary catheter. Most patients return home after a few hours, but some may need hospitalization for bleeding or other issues. The catheter is removed the day after surgery, unless bleeding is excessive or other concerns arise.

HoLEP surgery recoveryAfter surgery, individuals can walk, but they should avoid strenuous activities for two weeks and straddle activities for six weeks. Most can return to work within one to two weeks. However, it might take up to 12 weeks to completely recover.

Post-procedure care: Follow the doctor’s instructions for catheter cleaning, maintain a healthy diet, drink fluids, avoid strenuous activity, and strain for 2-4 weeks post-surgery to prevent infection and promote healing.

    FAQ's

HoLEP surgery indications include benign prostatic hyperplasia, failure of the conservational therapies for enlarged prostates, prostatic abnormalities, and more.

Following HoLEP surgery, patients may experience complete relief from BPH symptoms, but recovery time may take a few weeks, and strict post-surgical care and instructions must be followed as per the surgeon’s instructions for a few initial days.

One can return to normal activities within 2 weeks. However, complete recovery might take place around 6-12 weeks.

TURP, or transurethral resection of the prostate, is a surgical procedure for BPH patients, similar to HoLEP. The main difference between the two is the removal method, which impacts recovery time, risks, and costs. HoLEP is less likely to require blood transfusions or additional surgery due to prostate regrowth. However, the best prostate surgery depends on the individual’s condition and the advice of their urologist.

HoLEP is a minimally invasive surgery with low complications compared to other treatments for BPH, but it’s important to discuss any concerns with your urologists about the procedure and post-procedure recovery.

Based on the extent of the procedure and its severity, the HoLEP procedure lasts between 1 and 3 hours.
