    Hemiglossectomy Surgery Steps

    1.Preoperative Evaluation

A complete physical evaluation, along with blood and urine samples, is taken for analysis to determine the extent of the malignancy. Various assessments, such as X-rays, CT scans of the neck and liver, and biochemistry studies, are conducted   to ensure the overall accuracy of the surgery.

    2.Hemiglossectomy Procedure

    Hemiglossectomy steps include:

    • Positioning the patient in the most optimal surgically aseptic and comfortable position under general anaesthesia.
    • Incisions are marked, and the affected area is surgically removed, ensuring the tongue’s mobility is preserved.
    •  In cases of extensive resections,  the nearby skin flap is surgically connected  along with the blood vessels that supply it.
    • The flap is then inserted into the hole in the tongue, requiring a highly skilled surgeon capable of connecting very small arteries. This completes the reconstruction of the surgically operated area.
    • During and after the surgery, you will require a tracheotomy to facilitate breathing.

    3. Postoperative Care:

While recovering at the hospital, you may receive supplemental oxygen for the first 1-2 days. Oxygen is administered through a face mask or through two small tubes placed in the nostrils. Fluids and medications will be given through an IV.

    Hemiglossectomy Complications

Although hemiglossectomy is a generally safe and effective procedure, potential complications may include bleeding, infection, changes in speech and swallowing, and cosmetic concerns.

Recovery from hemiglossectomy involves several stages where follow-up care is of utmost importance for guiding back the patient into normalcy.

    4. Follow-Up Care:

    • Regular check-ups monitoring for any signs of infection or complications
    • Speech and Swallowing Therapy
    • Rehabilitation to adapt to changes in speech and swallowing.
    • Working with a speech therapist for optimal recovery.
    • Continued support for any speech or swallowing challenges.

    FAQ's

The choice of flap depends on various factors, including the size of the defect, the patient’s overall health, and the surgeon’s expertise. The radial forearm free flap, anterolateral thigh flap, and scapular flap are the most commonly used flaps.

The most frequent reason for hemiglossectomy is the presence of malignant tumours on one side of the tongue. If the tumour is extensive or cannot be effectively treated with less invasive methods, a hemiglossectomy may be deemed necessary.

Half of the tongue, adjacent lymph nodes, surrounding soft tissues, and part of the mandible ( if involved) and, depending on the extent of the disease, adjacent tissues or structures may be excised to ensure complete removal of the affected tissue.

The surgery involves dissolvable stitches, which are made from materials that the body gradually absorbs over time. The timeframe for dissolvable stitches to stay in place can range from a few days to a few weeks, depending on the type of stitches used.

Eating and speaking after a tongue surgery requires careful consideration to promote healing and minimise discomfort. Speech therapists and our healthcare team provide personalised guidance based on your specific condition and recovery progress.
