    Bullectomy Overview:

Bullectomy is a surgical intervention aimed at removing enlarged air sacs, known as bullae, from the lungs. These bullae typically measure about 1 cm in diameter when distended and can expand up to 20 cm. Commonly associated with COPD and emphysema, bullae can lead to a collapsed lung if they rupture, necessitating bullectomy if recurrent collapses occur. This procedure is typically performed by thoracic surgeons or skilled pulmonologists, especially when breathing difficulties arise due to complications. Apart from addressing COPD and emphysema-related issues, bullectomy can also be beneficial in treating conditions such as Ehlers-Danlos syndrome, Marfan syndrome, sarcoidosis, and HIV-associated emphysema.

    Bullectomy types

Bullectomy is mainly categorized into two types: thoracotomy (an open surgery) and VATS (video-assisted thoracoscopic surgery). Advancements in these surgical techniques have led to variations within these main categories, providing additional benefits with procedures such as laser bullectomy, video-assisted thoracoscopic stapling (VATS stapling), and robotic-assisted bullectomy.

    Procedure Name Bullectomy
    Type of Surgery Minimally invasive
    Type of Anesthesia General Anesthesia
    Procedure Duration 1-3 hours
Recovery Duration Few days to few weeks
    Bullectomy: Pre-Op & Post-Op Care

Preparation: Bullectomy preparation involves a medical history evaluation, physical examination, and discussion of procedure details, including suitable surgery (VATS or thoracotomy) based on the patient’s health and lung function.

During the procedure: A bullectomy can be performed through a thoracotomy (15–20 cm incision) or video-assisted thoracoscopic surgery (3–4 cm incision), with the latter being minimally invasive. The doctor will recommend the most suitable approach based on the bull’s location and size. Both techniques require general anesthesia. After the surgery, the cuts are closed with stitches, and a tube is inserted into the chest to drain air and fluids, which helps in recovery.

After the procedure: After a bullectomy, expect a 1-2 day hospital stay, pain management with medication, breathing exercises, and treatments, and a recovery period of several weeks with limited strenuous activity.

Bullectomy recoveryRecovery time for surgery varies, typically 2-3 days, with full recovery after a few weeks of rest. Appetite and bowel movements may take time to return.

Post-procedure care: Doctors advise avoiding air travel for 3 months after surgery, as the healing process can take time, and individuals should not rush to return immediately to normal activities until cleared by the surgeon.

    FAQ's

Bullectomy indications include removal of bullae, dyspnea (shortness of breath), exercise intolerance, pneumothorax, infections, chest pain, exacerbations, cough with mucus, airway obstructions, etc.

The recovery time might vary from person to person based on the age, severity of the condition, and overall health condition. Usually, the recovery for a bullectomy might be anywhere between 2–3 days, or else up to a few weeks in some cases.

A bullectomy can improve lung and respiratory health, reduce dyspnea, lower the risk of consequences including serious infections, remove the bullae to improve overall health, and more.

Bullectomy in general may feel uncomfortable, particularly during the early stages of recovery. Usually, taking pain medication helps to ease this discomfort. When compared to open surgery, less invasive VATS bullectomy—which is frequently utilized for smaller bullae—may cause less pain following surgery. Nevertheless, the patient will receive anesthesia to eliminate the pain during the procedure.

Lung bullae are usually treated by bullectomy surgery. Large lung bullae are treated by the open bullectomy whereas the small bullae are treated by the VATS or minimally invasive procedures.

Enlarging aberrant lung air sacs are known as a bulla. On its own, a lung bullae is not cancerous. However, in some rare cases, these bullae might become cancerous.
