Background

A 12-year-old female had congenital heart disease, atrial septal defect, mitral and tricuspid regurgitation, RV dysfunction, and severe PAH.

Diagnosis and Treatment

Echocardiography and cardiac catheterization showed significant left to right shunt with PVRI of 2.32 WU/sq.m. She was optimized for corrective open surgery involving MV repair – anterolateral commissural plication and posterior annuloplasty, TV repair – 32 CE Classic ring annuloplasty, and ASD closure with perforated autologous pericardial patch. The patient remained on LV support until the second postoperative day (PO2) and shifted to ward on the fifth postoperative day (PO5)
