Successful Management of Carotid Artery Stenosis Complicated by Neurological Deficits

Patient Profile:

A 60-year-old female presented with complaints of left upper and lower limb weakness, with a power of 3/5. Imaging revealed an ulcerated atheromatous plaque causing 80-90% stenosis in the internal carotid artery. Over time, the patient’s weakness progressed to 0/5, accompanied by aphasia, possibly due to hypoperfusion.

Diagnostic Imaging:

DSA and MRI scans confirmed the presence of an ulcerated atheromatous plaque, causing significant stenosis in the internal carotid artery.

Clinical Progression:

The patient’s condition deteriorated, with worsening weakness and aphasia, indicating compromised perfusion to the brain.

Intervention:

An emergency carotid endarterectomy with vein patch plasty was performed promptly to address the carotid artery stenosis and restore adequate blood flow to the brain.

Outcome:

Following the procedure, the patient experienced complete recovery of neurological deficits and was discharged in a stable condition.
