Background

24 year old man presented with acute onset right second fingertip gangrene.

Diagnosis & Treatment

On evaluation he was found to have arterial thoracic outlet syndrome with right complete cervical rib. He was initially managed with anticoagulation and later taken up for Thoracic Outlet Decompression. Intraoperatively the patient was found to have scalene muscle hypertrophy and a complete cervical rib fused to a broad first rib causing subclavian artery compression. Patient had a successful outcome following the decompression procedure.
